Support Coordinator Assistant

Who we are: Arkansas Support Network is a licensed service provider that serves the state of Arkansas, with offices in Springdale, Fort Smith, Camden, and Jonesboro. We work with a wide range of government agencies and departments, nonprofit organizations, health care facilities, education agencies, local schools, foundations, businesses, and individuals to provide the most effective and least restrictive supports for people with disabilities in environments they and their families choose for themselves.

    What You'll Do:

  • Assist with implementing individualized service plans based on the unique needs and preferences of each individual.
  • Assist with implementing individualized service plans based on the unique needs and preferences of each individual.
  • Collaborate with individuals, their families, and support teams to execute comprehensive support plans created using the Charting the Life Course framework.
  • Monitor resources efficiently to meet the needs outlined in the individual's service plans. 4. Work collaboratively, assisting the Personnel team with monitoring SL scheduling and overtime within the coordinating division.
  • Conducting monthly home visits, 60-day progress reviews, and reviewing/approving DSP daily activity records.
  • Collaborate with various stakeholders, including healthcare providers, social services agencies, and community organizations. 
  • Establish and maintain effective communication with individuals, families, and support teams as the liaison and point of contact.
  • Maintain accurate and up-to-date documentation of services provided. 
  • Work collaboratively and cooperatively with departments and teams to facilitate agency compliance responsibilities and areas of improvement.
  • Establishing a strong and supportive relationship with the individuals and families receiving support. Serve as a point of contact and resource, coordinating all aspects of support, including crisis and emergency situation management for the person receiving services.
  • Other duties and responsibilities as assigned may vary depending on the individual and the agency’s needs.
  • What You Need: 

  • High School Diploma
  • Minimum 3+ verifiable years of experience 
  • Strong understanding of the agency's programs
  • Ability to develop people-focused relationships

 Pay Range: $16.35 - $18.75/hourly | Office Location: Springdale, Arkansas | Full-time position
